Payment is due at the time of service. If you choose to discontinue care before treatment is complete, you will receive a refund less the cost of care received.
Other arrangements may be made with the office depending upon special circumstances. An estimate of the charges for any procedure or surgery you may require will be provided prior to treatment. If you have any dental insurance we will be happy to assist you in filling out the proper forms, and we will work with your carrier to maximize your benefits. We will be happy to assist you in processing your dental insurance, even if we are not contracted with your company to a reasonable extent.
Expected insurance payments are an estimate only, even if we have directly contacted your insurance provider. Most insurance companies will respond within four to six weeks. Please call our office if your monthly statement does not reflect your expected insurance payment within that time frame. Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some companies pay fixed allowances for certain procedures and others pay a percentage of a charge. It is your responsibility to pay any deductible amount, co-insurance or any other balance not paid by your insurance company. Any balance remaining after your insurance has responded is your responsibility. Your prompt remittance is appreciated. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage, or the timing of their payment. Finance charges will be charged if your balance has not been paid through the initial insurance response. Accounts with balances active for longer than ninety days may be sent to collections. Account credits and overpayments will be returned after all treatment and corresponding payments from all parties have be completed. Unclaimed credits may be forfeited ninety days after account inactivity, unless prior arrangements have been made.
Accepting this financial policy allows our billing staff to contact you via all contact information you have provided, including mobile and landline phones. If you do not wish to be contacted by mobile phone, or by any other of the contact locations you have provided, please advise us of your preferred method of contact and provide written notice that you do not wish to be contacted by cellular phone or another specific point of contact.
